Orthobiologics: A Surgical Alternative
If you’re looking for nonsurgical options for common injuries, consider orthobiologics. Platelet-rich plasma (PRP), bone marrow aspirate (BMA), and adipose grafts are alternatives.
ORTHOBIOLOGICS FOR ARTHRITIS
Conventional arthritis treatment frequently includes steroid injections which can accelerate the progression of arthritis, leading to earlier need for joint replacement surgery. PRP, BMA, and adipose grafts are non-surgical options which do not accelerate the progression of arthritis and may actually slow down the rate of arthritis progression. Several recently published studies have demonstrated that PRP is superior to steroid and Hyaluronic Acid (aka viscosupplement, gel, rooster comb) injections in degree and duration of pain relief.
ADDRESSING THE PATHOLOGY OF CHRONIC TENDON PAIN
Anti-inflammatory treatments do not treat chronic tendon pain because it is not caused by inflammation. Orthobiologic interventions utilize cells from your own body to address the underlying source of your pain.
A COMMITMENT TO ETHICAL AND RESPONSIBLE ORTHOBIOLOGICS
Orthobiologics is a relatively new field of medicine. And like many new and exciting advances in science and medicine, there can be misinformation about use and outcomes. I provide Orthobiologics in line with the AMSSM "Responsible use of Orthobiologics" position statement, which includes:
- I participate in ongoing physician education and training on orthbiologic procedures.
- I communicate realistic expectations and outcomes based on current research with you.
- I evaluate PRP products produced for each patient to confirm the quality of the product and document PRP contents for tracking outcomes, adding to existing medical research.
- I participate in a national outcome-tracking registry, OBERD, to expand the medical community’s knowledge of these relatively new procedures.

Ultrasound-Guided Tenotomy is a minimally invasive technique used to treat chronic tendon and plantar fascia disease at its source. Dr. Miars uses the Tenex device when performing tenotomy procedures to treat tendon conditions like tennis elbow, patellar tendonitis, Achilles tendonitis, plantar fasciitis, and hip bursitis.

SPORTS INJURY APPLICATIONS FOR ORTHOBIOLOGICS
Orthobiologics is a subset of regenerative medicine. Regenerative medicine uses the body's own cells to treat, repair, or regrow tissues UCL. In orthobiologics, the focus is on platelet-rich plasma (PRP) to treat tendonitis, joint pain, and other common sports injuries in a less invasive way.
Sports Medicine Specialist with Certification in Musculoskeletal Ultrasound
A major component of Dr. Miars’ practice includes the use of ultrasound to aid in diagnosis and to direct minimally invasive interventions for sports injuries and orthopedic conditions. Dr. Miars has over 10 years of experience performing diagnostic and interventional MSKUS. He has provided ultrasound education for the Waco Sports Medicine Fellowship and served as faculty for the Regenerative Medicine Training Institute.
A POWERFUL, LESS EXPENSIVE DIAGNOSTIC TOOL
MSKUS is excellent at visualizing tendon abnormalities and is far less expensive than MRI. For many conditions, MSKUS provides the needed diagnostic information and a significantly lower cost.

MSKUS is also used to guide minimally invasive treatment for tendon issues. MSKUS is vital for Tenex techniques. Shorten recovery time for tennis elbow, jumper’s knee, plantar fasciitis, Achilles tendonitis and other chronic tendon injuries, carpal tunnel, and more.
